The NM_000527.5(LDLR):c.2389G>T (p.Val797Leu) variant causes a missense, splice region change involving the alteration of a non-conserved nucleotide. The gene LDLR is a known oncogene (CancerMine: 2 oncogene citations). The variant allele was found at a cumulative frequency of 0.00000186 (AC=3) in the gnomAD database across 1,613,688 control chromosomes (no homozygotes observed). The grpmax filtering allele frequency (95% CI) is 0.0000147. Variant has been reported in ClinVar as Pathogenic/Likely Pathogenic (★★). ClinVar reports functional evidence for this variant: "SCV000948390: RNA analysis indicates that this missense change induces altered splicing and likely results in a shortened protein product. PMID:19411563". A different missense at the same amino acid position has been reported as Pathogenic/Likely Pathogenic in ClinVar: p.V797M: Pathogenic/Likely_pathogenic (ClinVar VariationId 226393, 2 stars) Other variants at the same amino acid position have been reported in ClinVar (not pathogenic): p.V797= (synonymous): Conflicting_classifications_of_pathogenicity (ClinVar VariationId 891738); p.V797= (synonymous): Likely_benign (ClinVar VariationId 1094170, 1 star)

Genes affected
LDLR (HGNC:6547): (low density lipoprotein receptor) The low density lipoprotein receptor (LDLR) gene family consists of cell surface proteins involved in receptor-mediated endocytosis of specific ligands. The encoded protein is normally bound at the cell membrane, where it binds low density lipoprotein/cholesterol and is taken into the cell. Lysosomes release the cholesterol, which is made available for repression of microsomal enzyme 3-hydroxy-3-methylglutaryl coenzyme A (HMG CoA) reductase, the rate-limiting step in cholesterol synthesis. At the same time, a reciprocal stimulation of cholesterol ester synthesis takes place. Mutations in this gene cause the autosomal dominant disorder, familial hypercholesterolemia. Alternate splicing results in multiple transcript variants.[provided by RefSeq, May 2022]

Classification according to ACGS-UK Somatic Oncogenicity v2025

Classification was made for transcript:

Our verdict: Uncertain_significance. The variant received 4 points.

O3Absent or very rare in population
Very rare in gnomAD (popmax AF <0.01%) — O3 supporting; GnomAD effective popmax AF = 0.000015 — very rare (O3 supporting [+1])
O6Computational evidence supports deleterious effect
Computational evidence supports a deleterious/oncogenic effect; Missense variant — primary computational scorer does not indicate an oncogenic effect; O6 not met via missense path; Splice-region variant — splicing algorithms predict an impact on splicing; supports O6
O7Mutational hotspot or critical domain
Supporting: cancerhotspots.org hit, critical domain, or missense neighbourhood hotspot; No cancerhotspots.org hit at this position; UniProt domain: 0 pathogenic, 0 benign.; ±8 AA neighbourhood: 6 pathogenic, 3 benign (OR vs. background: 9.3); No cancerhotspots.org splice hotspot hit at this position
O8Missense constraint in gene or region
Missense in gene/domain constrained for missense variation where missense is common disease mechanism (O8 supporting); Gene/disease mechanism: missense is a common mechanism of oncogenesis; gnomAD missense Z-score: 0.61 (threshold ≥3.09 for constraint); Domain-level constraint: variant is in a critically constrained functional domain (IS_AT_CRITICAL_DOMAIN)
