Mutations in this gene are associated with malignant hyperthermia susceptibility, central core disease, and minicore myopathy with external ophthalmoplegia. The c.13690C>T (p.R4564W) alteration is located in exon 94 (coding exon 94) of the RYR1 gene. This alteration results from a C to T substitution at nucleotide position 13690, causing the arginine (R) at amino acid position 4564 to be replaced by a tryptophan (W)._x000D_ Based on the available evidence, the RYR1 c.13690C>T (p.R4564W) alteration is classified as likely pathogenic for autosomal recessive RYR1-related myopathy; however, its clinical significance for autosomal dominant RYR1-related myopathy and autosomal dominant malignant hyperthermia susceptibility is uncertain. Based on data from gnomAD, the T allele has an overall frequency of 0.004% (9/251490) total alleles studied. The highest observed frequency was 0.02% (7/34592) of Latino alleles. This variant has been detected in trans with a second RYR1 variant in multiple individuals with clinical features of RYR1-related myopathy (Ambry internal data; external communications; Juntas Morales, 2021). This amino acid position is highly conserved in available vertebrate species. This alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, this alteration is classified as likely pathogenic. -

This sequence change replaces arginine, which is basic and polar, with tryptophan, which is neutral and slightly polar, at codon 4564 of the RYR1 protein (p.Arg4564Trp). This variant is present in population databases (rs753208767, gnomAD 0.02%). This missense change has been observed in individual(s) with autosomal recessive RYR1-related conditions (PMID: 29792937; Invitae). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. ClinVar contains an entry for this variant (Variation ID: 282726). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt RYR1 protein function with a positive predictive value of 95%. This variant disrupts the p.Arg4564 amino acid residue in RYR1. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 18253926, 30611313; Invitae). This suggests that this residue is clinically significant, and that variants that disrupt this residue are likely to be disease-causing. For these reasons, this variant has been classified as Pathogenic. -

To our knowledge, functional studies have not been reported for this variant. This variant has not been reported in individuals affected with autosomal dominant malignant hyperthermia in the literature, although it is associated with other phenotype(s) (ClinVar variation ID: 282726). This variant has been identified in 9/251490 chromosomes in the general population by the Genome Aggregation Database (gnomAD). It has been previously reported in an individual undergoing genetic testing for a myopathy or muscular dystrophy phenotype (Zenagui et al., 2018). This variant is reported with the allele frequency (0.003%) in the gnomAD and novel in 1000 genome database. The amino acid Arg at position 4564 is changed to a Trp changing protein sequence and it might alter its composition and physico-chemical properties. The amino acid change p.Arg4564Trp in RYR1 is predicted as conserved by GERP++ and PhyloP across 100 vertebrates. For these reasons, this variant has been classified as Variant of Uncertain Significance (VUS). -
