Variant summary: FKRP c.169G>A (p.Glu57Lys) results in a conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 8.6e-06 in 231428 control chromosomes. c.169G>A has been reported in the presumed compound heterozygous state in the literature in at least 2 individuals affected with Limb-Girdle Muscular Dystrophy, Autosomal Recessive (example, Awano_2021, Nallamilli_2018). These data indicate that the variant may be associated with disease. At least one publication reports experimental evidence evaluating an impact on protein function in vitro. The most pronounced variant effect results in 10%-<30% of normal activity (example, Ma_2024). The following publications have been ascertained in the context of this evaluation (PMID: 34509255, 39326416, 30564623). ClinVar contains an entry for this variant (Variation ID: 283305). Based on the evidence outlined above, the variant was classified as likely pathogenic. -

For these reasons, this variant has been classified as Pathogenic. ClinVar contains an entry for this variant (Variation ID: 283305). This missense change has been observed in individual(s) with limb-girdle muscular dystrophy (PMID: 30564623, 32351701, 34509255). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. The frequency data for this variant in the population databases is considered unreliable, as metrics indicate poor data quality at this position in the gnomAD database. This sequence change replaces glutamic acid, which is acidic and polar, with lysine, which is basic and polar, at codon 57 of the FKRP protein (p.Glu57Lys). -
