The p.R150L variant (also known as c.449G>T), located in coding exon 4 of the BARD1 gene, results from a G to T substitution at nucleotide position 449. The arginine at codon 150 is replaced by leucine, an amino acid with dissimilar properties. This variant was detected in a cohort of 108 moderate to high-risk Malaysian breast cancer patients (Ng PS et al. Clin Genet, 2016 Oct;90:315-23). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear.

This sequence change replaces arginine, which is basic and polar, with leucine, which is neutral and non-polar, at codon 150 of the BARD1 protein (p.Arg150Leu). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with BARD1-related conditions (PMID: 26757417). ClinVar contains an entry for this variant (Variation ID: 926872). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.
