Genes affected
CYP1B1 (HGNC:2597): (cytochrome P450 family 1 subfamily B member 1) This gene encodes a member of the cytochrome P450 superfamily of enzymes. The cytochrome P450 proteins are monooxygenases which catalyze many reactions involved in drug metabolism and synthesis of cholesterol, steroids and other lipids. The enzyme encoded by this gene localizes to the endoplasmic reticulum and metabolizes procarcinogens such as polycyclic aromatic hydrocarbons and 17beta-estradiol. Mutations in this gene have been associated with primary congenital glaucoma; therefore it is thought that the enzyme also metabolizes a signaling molecule involved in eye development, possibly a steroid. ACMG classification
Classification made for transcript
Verdict is Pathogenic. Variant got 12 ACMG points.
PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant is located in the 3'-most exon, not predicted to undergo nonsense mediated mRNA decay. There are 40 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5

Congenital glaucoma Pathogenic:1
Pathogenic, criteria provided, single submitter | clinical testing | Labcorp Genetics (formerly Invitae), Labcorp | Apr 25, 2017 | A different truncation downstream of this variant (p.Asp449Metfs*8) has been determined to be pathogenic (PMID: 9497261, 27820421, 14635112, 12036985). This suggests that deletion of this region of the CYP1B1 protein is causative of disease. This variant has been reported in an individual affected with primary congenital glaucoma (PMID: 16735994). For these reasons, this variant has been classified as Pathogenic. This sequence change results in a premature translational stop signal in the last exon of the CYP1B1 mRNA at codon 434 (p.Trp434*). While this is not anticipated to result in nonsense mediated decay, it is expected to delete the last 110 amino acids (~20%) of the CYP1B1 protein. - |
