Genes affected
STK4 (HGNC:11408): (serine/threonine kinase 4) The protein encoded by this gene is a cytoplasmic kinase that is structurally similar to the yeast Ste20p kinase, which acts upstream of the stress-induced mitogen-activated protein kinase cascade. The encoded protein can phosphorylate myelin basic protein and undergoes autophosphorylation. A caspase-cleaved fragment of the encoded protein has been shown to be capable of phosphorylating histone H2B. The particular phosphorylation catalyzed by this protein has been correlated with apoptosis, and it's possible that this protein induces the chromatin condensation observed in this process. ACMG classification

Classification made for transcript

Verdict is Likely_pathogenic. Variant got 8 ACMG points.

PVS1
Loss of function variant, product does not undergo nonsense mediated mRNA decay. Variant located near the start codon (<100nt), not predicted to undergo nonsense mediated mRNA decay. Fraction of 0.936 CDS is truncated, and there are 0 pathogenic variants in the truncated region.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 20-44972133-G-GA is Pathogenic according to our data. Variant chr20-44972133-G-GA is described in ClinVar as [Pathogenic]. Clinvar id is 3727861.Status of the report is criteria_provided_single_submitter, 1 stars.

This sequence change creates a premature translational stop signal (p.Asp31Glufs*20) in the STK4 gene. It is expected to result in an absent or disrupted protein product. Loss-of-function variants in STK4 are known to be pathogenic (PMID: 22174160). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with STK4-related conditions. For these reasons, this variant has been classified as Pathogenic. -
