The arginine at codon 266 is replaced by tryptophan, an amino acid with dissimilar properties. This alteration has been detected in trans with another SLC52A3 variant in an individual with overlapping features of Brown-Vialetto-Van Laere syndrome 1 (BVVLS1) and Fazio-Londe disease (Ciccolella M et al. Neuromuscul Disord, 2012 Dec;22:1075-82). This amino acid position is not well conserved in available vertebrate species, and tryptophan is the reference amino acid in other vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |

This variant is observed in 5/126660 (0.0039%) alleles from individuals of non-Finnish European background in large population cohorts, with no homozygotes identified (Lek et al., 2016). The R266W variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. In-silico analyses, including protein predictors and evolutionary conservation, support that this variant does not alter protein structure/function.We interpret R266W as a variant of uncertain significance. - |
