KCNE1 (HGNC:6240): (potassium voltage-gated channel subfamily E regulatory subunit 1) The product of this gene belongs to the potassium channel KCNE family. Potassium ion channels are essential to many cellular functions and show a high degree of diversity, varying in their electrophysiologic and pharmacologic properties. This gene encodes a transmembrane protein known to associate with the product of the KVLQT1 gene to form the delayed rectifier potassium channel. Mutation in this gene are associated with both Jervell and Lange-Nielsen and Romano-Ward forms of long-QT syndrome. The p.Val80Ile variant in KCNE1 has not been previously reported in individuals with hearing loss or Jervell and Lange-Nielson syndrome, but has been identified in one individual with long QT syndrome (Riuro 2015) and in 4/66620 of African chromosomes by the Exome Aggregation Consortium (ExAC, http://exac.broadinstitut e.org; dbSNP rs769368494). Although this variant has been seen in the general po pulation, its frequency is not high enough to rule out a pathogenic role. Comput ational prediction tools and conservation analyses suggest that the p.Val80Ile v ariant may not impact the protein, though this information is not predictive eno ugh to rule out pathogenicity. In summary, the clinical significance of the p.Va l80Ile variant is uncertain. -

This variant is present in population databases (rs769368494, gnomAD 0.03%). This missense change has been observed in individual(s) with long QT syndrome (PMID: 24667783, 39082327). ClinVar contains an entry for this variant (Variation ID: 228762). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt KCNE1 protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.V80I variant (also known as c.238G>A), located in coding exon 1 of the KCNE1 gene, results from a G to A substitution at nucleotide position 238. The valine at codon 80 is replaced by isoleucine, an amino acid with highly similar properties. This variant was detected in a long QT syndrome cohort; however, clinical details were not provided (Riuró H et al. Eur. J. Hum. Genet. 2015;23:79-85). This amino acid position is not well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
