CBS (HGNC:1550): (cystathionine beta-synthase) The protein encoded by this gene acts as a homotetramer to catalyze the conversion of homocysteine to cystathionine, the first step in the transsulfuration pathway. The encoded protein is allosterically activated by adenosyl-methionine and uses pyridoxal phosphate as a cofactor. Defects in this gene can cause cystathionine beta-synthase deficiency (CBSD), which can lead to homocystinuria. This gene is a major contributor to cellular hydrogen sulfide production. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 248910 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.1039G>T in individuals affected with Homocystinuria and no experimental evidence demonstrating its impact on protein function have been reported. No submitters have cited clinical-significance assessments for this variant to ClinVar after 2014. However, a different missense variant at this position has been classified as pathogenic previously (c.1039G>A [p.Gly347Ser], ClinVar:188801), indicating that Gly347 may of clinical significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -
