Variant summary

Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PM1PM2PM5PP3_StrongPP5_Very_Strong

The NM_000383.4(AIRE):c.278T>G(p.Leu93Arg) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant was absent in control chromosomes in GnomAD project. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. L93Q) has been classified as Likely pathogenic.

Genes affected
AIRE (HGNC:360): (autoimmune regulator) This gene encodes a transcriptional regulator that forms nuclear bodies and interacts with the transcriptional coactivator CREB binding protein. The encoded protein plays an important role in immunity by regulating the expression of autoantigens and negative selection of autoreactive T-cells in the thymus. Mutations in this gene cause the rare autosomal-recessive systemic autoimmune disease termed autoimmune polyendocrinopathy with candidiasis and ectodermal dystrophy (APECED). ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 18 ACMG points.

PM1
In a hotspot region, there are 7 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 0 benign, 5 uncertain in NM_000383.4
PM2
Very rare variant in population databases, with high coverage;
PM5
Other missense variant is known to change same aminoacid residue: Variant chr21-44286702-T-A is described in ClinVar as Conflicting_classifications_of_pathogenicity. ClinVar VariationId is 2501923.
PP3
MetaRNN computational evidence supports a deleterious effect, 0.992
PP5
Variant 21-44286702-T-G is Pathogenic according to our data. Variant chr21-44286702-T-G is described in ClinVar as Pathogenic/Likely_pathogenic. ClinVar VariationId is 68224.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

This variant is not present in population databases (gnomAD no frequency). For these reasons, this variant has been classified as Pathogenic. Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt AIRE protein function. ClinVar contains an entry for this variant (Variation ID: 68224). This variant is also known as c.398T>G. This missense change has been observed in individual(s) with autoimmune polyendocrinopathy syndrome (PMID: 10084559). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. It has also been observed to segregate with disease in related individuals. This sequence change replaces leucine, which is neutral and non-polar, with arginine, which is basic and polar, at codon 93 of the AIRE protein (p.Leu93Arg).
