CHEK2 (HGNC:16627): (checkpoint kinase 2) In response to DNA damage and replication blocks, cell cycle progression is halted through the control of critical cell cycle regulators. The protein encoded by this gene is a cell cycle checkpoint regulator and putative tumor suppressor. It contains a forkhead-associated protein interaction domain essential for activation in response to DNA damage and is rapidly phosphorylated in response to replication blocks and DNA damage. When activated, the encoded protein is known to inhibit CDC25C phosphatase, preventing entry into mitosis, and has been shown to stabilize the tumor suppressor protein p53, leading to cell cycle arrest in G1. In addition, this protein interacts with and phosphorylates BRCA1, allowing BRCA1 to restore survival after DNA damage. Mutations in this gene have been linked with Li-Fraumeni syndrome, a highly penetrant familial cancer phenotype usually associated with inherited mutations in TP53. Also, mutations in this gene are thought to confer a predisposition to sarcomas, breast cancer, and brain tumors. This nuclear protein is a member of the CDS1 subfamily of serine/threonine protein kinases. Several transcript variants encoding different isoforms have been found for this gene. The c.1260-8A>G intronic pathogenic mutation results from an A to G substitution 8 nucleotides upstream from coding exon 11 in the CHEK2 gene. This nucleotide position is well conserved in available vertebrate species. In silico splice site analysis predicts that this alteration will weaken the native splice acceptor site and will result in the creation or strengthening of a novel splice acceptor site. RNA studies have demonstrated that this alteration results in abnormal splicing in the set of samples tested (Ambry internal data; Kleiblova P et al. Int. J. Cancer, 2019 10;145:1782-1797). Based on the supporting evidence, this alteration is interpreted as a disease-causing mutation. -

This variant causes an A to G nucleotide substitution at the -8 position of intron 11 of the CHEK2 gene. RNA studies with multiple carrier individuals have shown that this variant creates a new splice acceptor site that results in the inclusion of 7 intronic nucleotides and, subsequently, frameshift and premature protein translation start signal (PMID: 31050813; ClinVar SCV002681983.1). This variant has been reported in an individual affected with familial breast cancer (PMID: 31050813) and in several unaffected individuals (PMID: 31050813). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). Loss of CHEK2 function is a known mechanism of disease (clinicalgenome.org). Based on the available evidence, this variant is classified as Likely Pathogenic. -
