CHEK2 (HGNC:16627): (checkpoint kinase 2) In response to DNA damage and replication blocks, cell cycle progression is halted through the control of critical cell cycle regulators. The protein encoded by this gene is a cell cycle checkpoint regulator and putative tumor suppressor. It contains a forkhead-associated protein interaction domain essential for activation in response to DNA damage and is rapidly phosphorylated in response to replication blocks and DNA damage. When activated, the encoded protein is known to inhibit CDC25C phosphatase, preventing entry into mitosis, and has been shown to stabilize the tumor suppressor protein p53, leading to cell cycle arrest in G1. In addition, this protein interacts with and phosphorylates BRCA1, allowing BRCA1 to restore survival after DNA damage. Mutations in this gene have been linked with Li-Fraumeni syndrome, a highly penetrant familial cancer phenotype usually associated with inherited mutations in TP53. Also, mutations in this gene are thought to confer a predisposition to sarcomas, breast cancer, and brain tumors. This nuclear protein is a member of the CDS1 subfamily of serine/threonine protein kinases. Several transcript variants encoding different isoforms have been found for this gene. This missense variant replaces proline with arginine at codon 85 of the CHEK2 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). A functional study has shown the mutant protein to exhibit partial loss of kinase activity (PMID: 22114986). This variant has been reported in an individual affected with familial breast cancer (PMID: 22114986) and a pediatric individual affected with Ewing sarcoma (PMID: 34771502). This variant has been identified in 3/251444 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

The p.P85R variant (also known as c.254C>G), located in coding exon 1 of the CHEK2 gene, results from a C to G substitution at nucleotide position 254. The proline at codon 85 is replaced by arginine, an amino acid with dissimilar properties. This variant has been identified in multiple individuals diagnosed with breast cancer (Desrichard A et al. Breast Cancer Res. 2011;13(6):R119; Greville-Heygate SL et al. JCO Precis Oncol, 2020 May;4:). This alteration also demonstrated reduced kinase activity compared to wild type in vitro (Desrichard A et al. Breast Cancer Res. 2011;13(6):R119). This alteration has also been reported in at least one subject in a study of 13087 breast cancer cases and 5488 control individuals in the UK (Decker B et al. J Med Genet, 2017 11;54:732-741). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. Experimental studies are conflicting or provide insufficient evidence to determine the effect of this variant on CHEK2 function (PMID: 22114986). RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (internal data). Variant summary: CHEK2 c.254C>G (p.Pro85Arg) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 1.6e-05 in 253842 control chromosomes (gnomAD). c.254C>G has been reported in the literature in both control individuals as well as individuals affected with Breast Cancer (Desrichard_2011, Decker_2017, Girard_2019, Greville-Heygate_2020, Dorling_2021). These reports do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. Experimental evidence demonstrated that the variant had intermediate activity, showing approximately 50% reduction in activity compared to wild-type (Desrichard_2011). Five ClinVar submitters have assessed the variant since 2014: all five classified the variant as uncertain significance.
