p.Gln4Arg in exon 2 of MYH9: This variant is not expected to have clinical sign ificance because although the glutamine (Gln) at position 4 is conserved in mamm als, it is not conserved in evolutionary distant species, with over 10 species ( birds, reptiles, fish) having an arginine (Arg). It has been identified in 3/85 94 East Asian chromosomes by the Exome Aggregation Consortium (ExAC, http://exac .broadinstitute.org; dbSNP rs773960235). In summary, this information indicates that this variant is likely benign.
The MYH9 c.11A>G (p.Gln4Arg) variant identified in this individual was previously classified as a variant of uncertain significance on the original report issued 09/09/2022. Since the original reporting of this variant, updated population data indicates that this variant is present in the population at an allele frequency higher than expected for a pathogenic variant. This variant has been identified in 16/44,892 East Asian chromosomes (69/1,614,046 chromosomes overall including 3 homozyous individuals) by the Genome Aggregation Database v4.1.0 (http://gnomad.broadinstitute.org/). This additional information provides sufficient evidence to update the classification of the MYH9 c.11A>G (p.Gln4Arg) variant to likely benign.
Variant summary: MYH9 c.11A>G (p.Gln4Arg) results in a conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be tolerated. The variant allele was found at a frequency of 3.2e-05 in 248786 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.11A>G in individuals affected with Macrothrombocytopenia And Granulocyte Inclusions With Or Without Nephritis Or Sensorineural Hearing Loss and no experimental evidence demonstrating its impact on protein function have been reported. ClinVar contains an entry for this variant (Variation ID: 505004). Based on the evidence outlined above, the variant was classified as uncertain significance.
not provided Benign:3
Inborn genetic diseases Uncertain:1
The c.11A>G (p.Q4R) alteration is located in exon 2 (coding exon 1) of the MYH9 gene. This alteration results from a A to G substitution at nucleotide position 11, causing the glutamine (Q) at amino acid position 4 to be replaced by an arginine (R). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear.
MYH9-related disorder Benign:1
This variant is classified as likely benign based on ACMG/AMP sequence variant interpretation guidelines (Richards et al. 2015 PMID: 25741868, with internal and published modifications).
