The encoded protein is involved in the ubiquitination and degradation of hypoxia-inducible-factor (HIF), which is a transcription factor that plays a central role in the regulation of gene expression by oxygen. In addition to oxygen-related gene expression, this protein plays a role in many other cellular processes including cilia formation, cytokine signaling, regulation of senescence, and formation of the extracellular matrix. Variants of this gene are associated with von Hippel-Lindau syndrome, pheochromocytoma, erythrocytosis, renal cell carcinoma, and cerebellar hemangioblastoma. [provided by RefSeq, Jun 2022]

Uncertain significance, criteria provided, single submitter | clinical testing | GeneDx | May 06, 2016 | This variant is denoted VHL c.440T>C at the cDNA level, p.Ile147Thr (I147T) at the protein level, and results in the change of an Isoleucine to a Threonine (ATT>ACT). This variant was observed in an individual with a unilateral pheochromocytoma (van der Harst 1998). VHL Ile147Thr was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, suggesting it is not a common benign variant in these populations. Since Isoleucine and Threonine differ in polarity, charge, size or other properties, this is considered a non-conservative amino acid substitution. VHL Ile147Thr occurs at a position that is not conserved and is located in the nuclear export/beta-domain as well as a region involved in binding to the CCT complex (Yuen 2009, UniProt). In silico analyses are inconsistent regarding the effect this variant may have on protein structure and function. Based on currently available evidence, it is unclear whether VHL Ile147Thr is pathogenic or benign. We consider it to be a variant of uncertain significance. - |

The isoleucine at codon 147 is replaced by threonine, an amino acid with similar properties. This variant has been detected in a patient with apparently sporadic pheochromocytoma (van der Harst E et al. Int J Cancer, 1998 Jul;77:337-40; Korpershoek E et al. Ann N Y Acad Sci, 2006 Aug;1073:138-48). In one functional study, I147T was not shown to impair microtubule stabilization (Hergovich A et al. Nat Cell Biol, 2003 Jan;5:64-70). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. - |
