This missense variant replaces isoleucine with threonine at codon 147 of the VHL protein. Computational prediction is inconclusive regarding the impact of this variant on protein structure and function (internally defined REVEL score threshold 0.5 < inconclusive < 0.7, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in at least one individual affected with pheochromocytoma (PMID: 9663592, 17102080). This variant has been identified in 1/251496 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.

This sequence change replaces isoleucine, which is neutral and non-polar, with threonine, which is neutral and polar, at codon 147 of the VHL protein (p.Ile147Thr). This variant is present in population databases (no rsID available, gnomAD 0.0009%). This missense change has been observed in individual(s) with sporadic pheochromocytoma (PMID: 9663592, 17102080). ClinVar contains an entry for this variant (Variation ID: 411964). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt VHL protein function with a positive predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

This variant is denoted VHL c.440T>C at the cDNA level, p.Ile147Thr (I147T) at the protein level, and results in the change of an Isoleucine to a Threonine (ATT>ACT). This variant was observed in an individual with a unilateral pheochromocytoma (van der Harst 1998). VHL Ile147Thr was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, suggesting it is not a common benign variant in these populations. Since Isoleucine and Threonine differ in polarity, charge, size or other properties, this is considered a non-conservative amino acid substitution. VHL Ile147Thr occurs at a position that is not conserved and is located in the nuclear export/beta-domain as well as a region involved in binding to the CCT complex (Yuen 2009, UniProt). In silico analyses are inconsistent regarding the effect this variant may have on protein structure and function. Based on currently available evidence, it is unclear whether VHL Ile147Thr is pathogenic or benign. We consider it to be a variant of uncertain significance.
Hereditary cancer-predisposing syndrome Uncertain:1
The p.I147T variant (also known as c.440T>C), located in coding exon 2 of the VHL gene, results from a T to C substitution at nucleotide position 440. The isoleucine at codon 147 is replaced by threonine, an amino acid with similar properties. This variant has been detected in a patient with apparently sporadic pheochromocytoma (van der Harst E et al. Int J Cancer, 1998 Jul;77:337-40; Korpershoek E et al. Ann N Y Acad Sci, 2006 Aug;1073:138-48). In one functional study, I147T was not shown to impair microtubule stabilization (Hergovich A et al. Nat Cell Biol, 2003 Jan;5:64-70). This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.
