Mutations in this gene are associated with Wolfram syndrome, also called DIDMOAD (Diabetes Insipidus, Diabetes Mellitus, Optic Atrophy, and Deafness), an autosomal recessive disorder. The disease affects the brain and central nervous system. Mutations in this gene can also cause autosomal dominant deafness 6 (DFNA6), also known as DFNA14 or DFNA38. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant allele was found at a frequency of 0.00018 in 250084 control chromosomes. This frequency is not significantly higher than estimated for a pathogenic variant in WFS1 causing Wolfram Syndrome 1, allowing no conclusion about variant significance. c.2054G>A has been observed in individual(s) affected with early-onset diabetes, without strong evidence for causality (Artuso_2015). These report(s) do not provide unequivocal conclusions about association of the variant with Wolfram Syndrome 1. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. A WFS1 c.2054G>A (p.Arg685His) variant was identified in a heterozygous state. This variant has been reported in a family with early-onset diabetes (Artuso R et al., PMID: 25048417). This variant has been reported as a germline variant of uncertain significance by several submitters in the ClinVar database (ClinVar Variation ID: 215395). It is observed in 49/281,476 alleles in the general population (gnomAD v2.1.1). Computational predictors indicate that the variant is damaging, evidence that correlates with impact to WFS1 function. Due to limited information, and based on ACMG/AMP guidelines for variant interpretation (Richards S et al., PMID: 25741868), the clinical significance of this variant is uncertain at this time.
