Variant summary
Our verdict is Pathogenic. The variant received 18 ACMG points: 18P and 0B. PM1PM2PM5PP3_StrongPP5_Very_Strong
The NM_003060.4(SLC22A5):c.847T>C(p.Trp283Arg) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.00000248 in 1,614,044 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Likely pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. W283C) has been classified as Likely pathogenic.

Variant summary: SLC22A5 c.847T>C (p.Trp283Arg) results in a non-conservative amino acid change located in the Major facilitator superfamily domain (IPR020846) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. Consensus agreement among computation tools predict no significant impact on normal splicing. However, these predictions have yet to be confirmed by functional studies. The variant was absent in 251492 control chromosomes. c.847T>C has been reported in the literature in an individual affected with Systemic Primary Carnitine Deficiency (Mayatepek_2000). Additionally another variant (c.847T>A) resulting in the same amino acid (p.Trp283Arg) has been observed in a homozygous individual (Fregeni_2017). At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect results in decreased transport activity (Mayatepek_2000, Fregeni_2017). The following publications have been ascertained in the context of this evaluation (PMID: 28841266, 12210323, 10612840).ClinVar contains an entry for this variant (Variation ID: 1424747). Based on the evidence outlined above, the variant was classified as likely pathogenic. -
For these reasons, this variant has been classified as Pathogenic. Algorithms developed to predict the effect of sequence changes on RNA splicing suggest that this variant may disrupt the consensus splice site. Experimental studies have shown that this missense change affects SLC22A5 function (PMID: 10612840). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt SLC22A5 protein function. ClinVar contains an entry for this variant (Variation ID: 1424747). This variant is also known as a T->C transition at nucleotide position (nt) 7305. This missense change has been observed in individual(s) with primary carnitine deficiency (PMID: 10612840). In at least one individual the data is consistent with being in trans (on the opposite chromosome) from a pathogenic variant. This variant is not present in population databases (gnomAD no frequency). This sequence change replaces tryptophan, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 283 of the SLC22A5 protein (p.Trp283Arg). -
