This variant is present in population databases (rs553257776, gnomAD 0.004%). This missense change has been observed in individuals with gastrointestinal stromal tumor and/or paragangliomas (PMID: 23282968, 33362715; Invitae). ClinVar contains an entry for this variant (Variation ID: 472395). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) has been performed at Invitae for this missense variant, however the output from this modeling did not meet the statistical confidence thresholds required to predict the impact of this variant on SDHA protein function. Experimental studies have shown that this missense change affects SDHA function (PMID: 28724664). This variant disrupts the p.Arg188 amino acid residue in SDHA. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 32741965; external communication). The arginine at codon 188 is replaced by tryptophan, an amino acid with dissimilar properties. This variant has been reported in individuals with a paraganglioma or gastrointestinal stromal tumor (Miettinen M et al, 2013 Feb;37:234-40; Ma X et al. Front Endocrinol (Lausanne), 2020 Dec;11:574662). Functional studies in yeast demonstrated that this variant impairs SDHA activity (Bannon AE et al. Clin Cancer Res, 2017 Nov;23:6733-6743). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis.
