The variant received 7 classification points (ACMG Germline Pathogenicity v2019). PM1PM2PM5PP3_ModerateBP6

The NM_000251.3(MSH2):c.2038C>G (p.Arg680Gly) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a cumulative frequency of 0.00000186 (AC=3) in the gnomAD database across 1,614,004 control chromosomes (no homozygotes observed). The grpmax filtering allele frequency (95% CI) is 0.0000241. In-silico predictor (REVEL) classifies this variant as likely damaging/oncogenic. Splicing prediction tools (SpliceAI) predict no significant impact on normal splicing. Variant has been reported in ClinVar as Benign/Likely Benign (no review stars). A different missense at the same amino acid position has been reported as Pathogenic/Likely Pathogenic in ClinVar: p.R680P: Pathogenic/Likely_pathogenic (ClinVar VariationId 483690, 2 stars) Other variants at the same amino acid position have been reported in ClinVar (not pathogenic): p.R680L: Uncertain_significance (ClinVar VariationId 1021808, 2 stars); p.R680Q: Uncertain_significance (ClinVar VariationId 579393, 2 stars); p.R680= (synonymous): Benign/Likely_benign (ClinVar VariationId 2827538, 2 stars); p.R680= (synonymous): Likely_benign (ClinVar VariationId 2819621, 1 star); p.R680= (synonymous): Likely_benign (ClinVar VariationId 4111043, 1 star)

Genes affected
MSH2 (HGNC:7325): (mutS homolog 2) This locus is frequently mutated in hereditary nonpolyposis colon cancer (HNPCC). When cloned, it was discovered to be a human homolog of the E. coli mismatch repair gene mutS, consistent with the characteristic alterations in microsatellite sequences (RER+ phenotype) found in HNPCC. Two transcript variants encoding different isoforms have been found for this gene. [provided by RefSeq, Apr 2012]

Classification according to ACMG Germline Pathogenicity v2019

Classification was made for transcript:

Our verdict: Likely_pathogenic. The variant received 7 points.

PM1Mutational hot spot or critical domain
Missense neighbourhood hotspot (≥2 P/LP within ±8 AA, OR ≥ 5 vs. background) — PM1; In-domain: 0 pathogenic, 0 benign rare missense variants.; ±8 AA neighbourhood: 19 pathogenic, 12 benign (OR vs. background: 7.8).
PM2Absent/extremely low in population databases
Very rare in gnomAD for AD/XL gene (popmax AF < threshold/10) — PM2; GnomAD popmax AF = 0.0000241 — very rare for AD+AR gene (base 0.0001, raised to 0.0005 by highest known P/LP ClinVar AF 0.00268) — PM2 moderate.
