This sequence change replaces asparagine, which is neutral and polar, with serine, which is neutral and polar, at codon 135 of the PSEN1 protein (p.Asn135Ser). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individuals with early-onset Alzheimer's disease (AD) (PMID: 15776278, 18580586, 23383383). It has also been observed to segregate with disease in related individuals. ClinVar contains an entry for this variant (Variation ID: 98022). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt PSEN1 protein function with a positive predictive value of 80%. This variant disrupts the p.Asn135 amino acid residue in PSEN1. Other variant(s) that disrupt this residue have been observed in individuals with PSEN1-related conditions (PMID: 9225696, 27793474), which suggests that this may be a clinically significant amino acid residue. For these reasons, this variant has been classified as Pathogenic. -

Variant summary: PSEN1 c.404A>G (p.Asn135Ser) results in a conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251472 control chromosomes. c.404A>G has been reported in the literature in multiple individuals affected with features of early onset Alzheimer Disease (Type 3) (example, Finckh_2005, Rudzinski_2009, Wojtas_2012, Ringman_2016). These data indicate that the variant is very likely to be associated with disease. The following publications have been ascertained in the context of this evaluation (PMID: 15776278, 33571524, 26888304, 18580586, 32105841, 23383383). Two submitters have cited clinical-significance assessments for this variant to ClinVar after 2014. All submitters classified the variant as pathogenic/likely pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -
