Genes affected
APC (HGNC:583): (APC regulator of WNT signaling pathway) This gene encodes a tumor suppressor protein that acts as an antagonist of the Wnt signaling pathway. It is also involved in other processes including cell migration and adhesion, transcriptional activation, and apoptosis. Defects in this gene cause familial adenomatous polyposis (FAP), an autosomal dominant pre-malignant disease that usually progresses to malignancy. Mutations in the APC gene have been found to occur in most colorectal cancers, where disease-associated mutations tend to be clustered in a small region designated the mutation cluster region (MCR) and result in a truncated protein product. Variant summary: The APC c.6658A>G (p.Asn2220Asp) variant involves the alteration of a conserved nucleotide and 3/4 in silico tools predict a benign outcome for this variant (SNPsandGO not captured due to low reliability index). However, these predictions have yet to be functionally assessed. This variant was found in 3/276652 control chromosomes (gnomAD) at a frequency of 0.0000108, which does not exceed the estimated maximal expected allele frequency of a pathogenic APC variant (0.0000714). The variant of interest has not, to our knowledge, been reported in affected individuals via publications. In addition, one clinical diagnostic laboratory classified this variant as uncertain significance. Because of the absence of clinical information and the lack of functional studies, the variant is classified as a "Variant of Uncertain Significance (VUS)," until additional information becomes available.

The p.Asn2220Asp variant in APC has not been previously reported in individuals with hereditary cancer or in large population studies, but has been identified i n 1/66412 European chromosomes by the Exome Aggregation Consortium (ExAC, http:/ /exac.broadinstitute.org; dbSNP rs374464049). Computational prediction tools and conservation analysis do not provide strong support for or against an impact to the protein. In summary, the clinical significance of the p.Asn2220Asp variant is uncertain.

This sequence change replaces asparagine, which is neutral and polar, with aspartic acid, which is acidic and polar, at codon 2220 of the APC protein (p.Asn2220Asp). This variant is present in population databases (rs374464049, gnomAD 0.002%). This missense change has been observed in individual(s) with APC associated conditions (PMID: 31422818). ClinVar contains an entry for this variant (Variation ID: 246324). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt APC protein function with a negative predictive value of 95%. RNA analysis performed to evaluate the impact of this missense change on mRNA splicing indicates it does not significantly alter splicing (internal data). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

This missense variant replaces asparagine with aspartic acid at codon 2220 of the APC protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual in a colorectal cancer study, however, it is not clear if the individual was affected or an unaffected control (PMID: 31422818). This variant has been identified in 2/281280 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance.

This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity.

The APC c.6658A>G variant is predicted to result in the amino acid substitution p.Asn2220Asp. This variant has been reported in an individual with colorectal cancer or colon polyps and classified as a variant of uncertain significance (Table S1, Gordon AS et al 2019. PubMed ID: 31422818). This variant is reported in 0.0016% of alleles in individuals of European (Non-Finnish) descent in gnomAD. A majority of submitters interpret this variant as uncertain in ClinVar (https://www.ncbi.nlm.nih.gov/clinvar/variation/246324/). At this time, the clinical significance of this variant is uncertain due to the absence of conclusive functional and genetic evidence.
