The p.S2794R variant (also known as c.8382C>A), located in coding exon 15 of the APC gene, results from a C to A substitution at nucleotide position 8382. The serine at codon 2794 is replaced by arginine, an amino acid with dissimilar properties. S2794 is in a putative phosphorylation site and mutagenesis of this amino acid in conjunction with S2793 to glutamates, a phosphomimetic, inhibits the C-terminal portion of APC from binding to EB proteins (Bu W, et al. J. Biol. Chem. 2003 Dec; 278(50):49721-31). This alteration was identified in one individual from a cohort of pancreatic cancer patients who also have a family history of pancreatic cancer (Chaffee KG et al. Genet. Med. 2018 01;20:119-127). This amino acid position is highly conserved in available vertebrate species. In addition, in silico predictors for this gene do not accurately predict pathogenicity. Missense alterations in APC are not a common cause of disease (Spier I et al. Genet Med. 2024 Feb;26(2):100992). Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
