Genes affected
ATM (HGNC:795): (ATM serine/threonine kinase) The protein encoded by this gene belongs to the PI3/PI4-kinase family. This protein is an important cell cycle checkpoint kinase that phosphorylates; thus, it functions as a regulator of a wide variety of downstream proteins, including tumor suppressor proteins p53 and BRCA1, checkpoint kinase CHK2, checkpoint proteins RAD17 and RAD9, and DNA repair protein NBS1. This protein and the closely related kinase ATR are thought to be master controllers of cell cycle checkpoint signaling pathways that are required for cell response to DNA damage and for genome stability. Mutations in this gene are associated with ataxia telangiectasia, an autosomal recessive disorder. [provided by RefSeq, Aug 2010]

The c.4612G>T variant (also known as p.V1538L) is located in coding exon 30 of the ATM gene. This change occurs in the first base pair of coding exon 30. This nucleotide position is highly conserved in available vertebrate species. In silico splice site analysis predicts that this alteration may weaken the native splice acceptor site. RNA studies have demonstrated that this alteration results in abnormal splicing in the set of samples tested (Ambry internal data). Based on the majority of available evidence to date, this variant is likely to be pathogenic. -

The missense variant NM_000051.4(ATM):c.4612G>T (p.Val1538Leu) causes a change at the same amino acid residue as a previously established pathogenic variant. The p.Val1538Leu variant is novel (not in any individuals) in gnomAD. The p.Val1538Leu variant is novel (not in any individuals) in 1kG. There is a small physicochemical difference between valine and leucine, which is not likely to impact secondary protein structure as these residues share similar properties. The gene ATM has a low rate of benign missense variation as indicated by a high missense variants Z-Score of 2.52.The p.Val1538Leu missense variant is predicted to be damaging by both SIFT and PolyPhen2. The valine residue at codon 1538 of ATM is conserved in all mammalian species. The nucleotide c.4612 in ATM is predicted conserved by GERP++ and PhyloP across 100 vertebrates. For these reasons, this variant has been classified as Likely Pathogenic -

Collection Method:clinical testing

This sequence change replaces valine, which is neutral and non-polar, with leucine, which is neutral and non-polar, at codon 1538 of the ATM protein (p.Val1538Leu). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with ATM-related conditions. ClinVar contains an entry for this variant (Variation ID: 524317). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be tolerated. Studies have shown that this missense change is associated with altered splicing resulting in multiple RNA products (Invitae). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Significance:Uncertain significance
Review Status:criteria provided, single submitter
Collection Method:clinical testing

The observed missense c.4612G>T(p.Val1538Leu) variant in ATM gene has not been reported previously as a pathogenic variant nor as a benign variant, to our knowledge. The p.Val1538Leu variant is absent in gnomAD Exomes database. This variant has been reported to the ClinVar database as Likely Pathogenic/ Uncertain significance. Multiple lines of computational evidence (Polyphen - possibly damaging , SIFT - damaging and MutationTaster - disease causing) predict a damaging effect on protein structure and function for this variant. The reference amino acid is predicted as conserved by GERP++ and PhyloP across 100 vertebrates. The amino acid Val at position 1538 is changed to a Leu changing protein sequence and it might alter its composition and physico-chemical properties. For these reasons, this variant has been classified as Uncertain Significance (VUS). -

Not observed in large population cohorts (Lek 2016); Has not been previously published as pathogenic or benign to our knowledge; While protein-based in silico analysis supports that this variant does not alter protein structure/function, splice predictors support a deleterious effect. However, in the absence of RNA or functional studies, the actual effect of this sequence change is unknown -

The ATM c.4612G>T (p.Val1538Leu) variant has been reported in the published literature in an individual with early-onset CRC (PMID: 31428572 (2019)). This variant has not been reported in large, multi-ethnic general populations (Genome Aggregation Database, http://gnomad.broadinstitute.org). Analysis of this variant using software algorithms for the prediction of the effect of nucleotide changes on splicing yielded predictions that this variant may affect proper ATM mRNA splicing. Based on the available information, we are unable to determine the clinical significance of this variant. -
