The NM_000053.4(ATP7B):c.3207C>A (p.His1069Gln) variant causes a missense change involving the alteration of a non-conserved nucleotide. The gene ATP7B is a known oncogene (CancerMine: 1 oncogene citations). The variant allele was found at a cumulative frequency of 0.000944 (AC=1,523) in the gnomAD database across 1,614,166 control chromosomes, including 1 homozygote. The grpmax filtering allele frequency (95% CI) is 0.00143. In-silico predictor (REVEL) classifies this variant as likely damaging/oncogenic. Splicing prediction tools (SpliceAI) predict no significant impact on normal splicing. Variant has been reported in ClinVar as Pathogenic/Likely Pathogenic (★★). ClinVar reports functional evidence for this variant: "SCV000052016: The most pronounced variant effect results in <10% of normal nucleotide binding activity (e.g., Morgan_2004). PMID:17717039, 10544227, 11690702, 15205462, 24897373, 21334398" and additional evidence is available in ClinVar. A different missense at the same amino acid position has been reported as Pathogenic/Likely Pathogenic in ClinVar: p.H1069L: Likely_pathogenic (ClinVar VariationId 2120118, 1 star) Other variants at the same amino acid position have been reported in ClinVar (not pathogenic): p.H1069= (synonymous): Likely_benign (ClinVar VariationId 2746502, 1 star) This exact variant is curated in the UniProt human variants database as Uncertain Significance.

Genes affected
ATP7B (HGNC:870): (ATPase copper transporting beta) This gene is a member of the P-type cation transport ATPase family and encodes a protein with several membrane-spanning domains, an ATPase consensus sequence, a hinge domain, a phosphorylation site, and at least 2 putative copper-binding sites. This protein is a monomer, and functions as a copper-transporting ATPase which exports copper out of the cells, such as the efflux of hepatic copper into the bile. Alternate transcriptional splice variants, encoding different isoforms with distinct cellular localizations, have been characterized. Mutations in this gene have been associated with Wilson disease which is characterized by copper accumulation. [provided by RefSeq, Dec 2019]

Classification according to ACGS-UK Somatic Oncogenicity v2025

Classification was made for transcript:

Our verdict: Uncertain_significance. The variant received 3 points.

O6Computational evidence supports deleterious effect
Computational evidence supports a deleterious/oncogenic effect; Missense variant — primary computational scorer supports an oncogenic/deleterious effect
O7Mutational hotspot or critical domain
Supporting: cancerhotspots.org hit, critical domain, or missense neighbourhood hotspot; No cancerhotspots.org hit at this position; UniProt domain: 0 pathogenic, 0 benign.; ±8 AA neighbourhood: 12 pathogenic, 0 benign (OR vs. background: 125.0); Variant does not impact splicing — splice-hotspot path not applicable
O8Missense constraint in gene or region
Missense in gene/domain constrained for missense variation where missense is common disease mechanism (O8 supporting); Gene/disease mechanism: missense is a common mechanism of oncogenesis; gnomAD missense Z-score: 0.84 (threshold ≥3.09 for constraint); Domain-level constraint: variant is in a critically constrained functional domain (IS_AT_CRITICAL_DOMAIN)
