Variant summary
Our verdict is Uncertain significance. The variant received 0 ACMG points: 2P and 2B. PM2BP4_Moderate
The NM_000057.4(BLM):c.3651A>T(p.Lys1217Asn) variant causes a missense change involving the alteration of a non-conserved nucleotide. The variant allele was found at a frequency of 0.00000274 in 1,461,812 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 13/22 in silico tools predict a benign outcome for this variant. Variant has been reported in ClinVar as Uncertain significance (★★). Synonymous variant affecting the same amino acid position (i.e. K1217K) has been classified as Likely benign.

Bloom syndrome Uncertain:2
This sequence change replaces lysine, which is basic and polar, with asparagine, which is neutral and polar, at codon 1217 of the BLM protein (p.Lys1217Asn). This variant is not present in population databases (gnomAD no frequency). This variant has not been reported in the literature in individuals affected with BLM-related conditions. ClinVar contains an entry for this variant (Variation ID: 127502). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt BLM protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

Hereditary cancer-predisposing syndrome Uncertain:2
The p.K1217N variant (also known as c.3651A>T), located in coding exon 18 of the BLM gene, results from an A to T substitution at nucleotide position 3651. The lysine at codon 1217 is replaced by asparagine, an amino acid with similar properties. This amino acid position is well conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

not provided Uncertain:1
This variant is denoted BLM c.3651A>T at the cDNA level, p.Lys1217Asn (K1217N) at the protein level, and results in the change of a Lysine to an Asparagine (AAA>AAT) in exon 19. This variant has not, to our knowledge, been published in the literature as either a mutation or a benign polymorphism, nor has it been observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Variant Server, indicating it is not a common benign variant in these populations. This variant is a non-conservative substitution of a positive polar amino acid for a neutral polar one, altering a position that is well conserved throughout evolution and is located in the Helicase and Rnase-D C-terminal domain. In silico analyses are inconsistent with regard to the effect this variant may have on protein structure and function. Based on the currently available information, we cannot assess whether this variant is benign or pathogenic, and therefore consider to be of unknown significance. The variant is found in HEREDICANCER panel(s). -
