The p.M815V variant (also known as c.2443A>G), located in coding exon 10 of the BRCA2 gene, results from an A to G substitution at nucleotide position 2443. The methionine at codon 815 is replaced by valine, an amino acid with highly similar properties. This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear.

This missense variant replaces methionine with valine at codon 815 of the BRCA2 protein. Computational prediction suggests that this variant may not impact protein structure and function. The BRCA2 c.2443A>G (p.Met815Val) variant has been reported in the published literature in individuals undergoing multigene panel testing due to personal or family history of cancer (PMID: 31853058 (2020)). In addition, it has been reported in a large population study screening for variants in BRCA1 and BRCA2 in Chinese individuals (PMID: 30702160 (2019)). This variant has also been described to be located in a region of the BRCA2 gene that is tolerant to missense sequence changes (PMID: 31911673 (2020)). The frequency of this variant in the general population, 0.0000041 (1/243410 chromosomes (Genome Aggregation Database, http://gnomad.broadinstitute.org)), is uninformative in the assessment of its pathogenicity. Analysis of this variant using bioinformatics tools for the prediction of the effect of amino acid changes on protein structure and function yielded predictions that this variant is benign. Based on the available information, we are unable to determine the clinical significance of this variant.

Five of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4.1e-06 in 243410 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. To our knowledge, no occurrence of c.2443A>G in individuals affected with Hereditary Breast And Ovarian Cancer Syndrome and no experimental evidence demonstrating its impact on protein function have been reported. At least one co-occurrence with another pathogenic variant has been reported (BRCA2 c.8053delA, p.Thr2685Hisfs*9; LOVD database), providing supporting evidence for a benign role. Four ClinVar submitters (evaluation after 2014) cite the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as VUS-possibly benign.
