The p.K817N variant (also known as c.2451G>T), located in coding exon 10 of the BRCA2 gene, results from a G to T substitution at nucleotide position 2451. The lysine at codon 817 is replaced by asparagine, an amino acid with similar properties. This amino acid position is poorly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -

Variant summary: BRCA2 c.2451G>T (p.Lys817Asn) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 8.2e-06 in 243484 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2451G>T has been reported in the literature in colorectal cancer cell lines (Ikediobi_2006, Mouradov_2014) and together with other BRCA2 and POLE variants of uncertain significance in a tumor from an individual with mixed endometrial and ovarian cancer (Villy_2021). These reports do not provide unequivocal conclusions about association of the variant with Hereditary Breast And Ovarian Cancer Syndrome. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Four clinical diagnostic laboratories have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All laboratories classified the variant as uncertain significance. Based on the evidence outlined above, the variant was classified as uncertain significance. -

This sequence change replaces lysine, which is basic and polar, with asparagine, which is neutral and polar, at codon 817 of the BRCA2 protein (p.Lys817Asn). This variant is present in population databases (rs767686668, gnomAD 0.002%). This missense change has been observed in individual(s) with clinical features of BRCA2-related conditions (PMID: 34541275). ClinVar contains an entry for this variant (Variation ID: 481190). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
