Hereditary cancer-predisposing syndrome Uncertain:1Benign:2
This missense variant replaces isoleucine with phenylalanine at codon 1173 of the BRCA2 protein. Computational prediction suggests that this variant may not impact protein structure and function. To our knowledge, functional studies have not been reported for this variant. This variant has been reported in three individuals affected with breast or ovarian cancer (PMID: 32380732, 34178674). A multifactorial analysis has reported a likelihood ratio for pathogenicity based on personal and family history of 1.684 from log(LR)=0.226 for one carrier (PMID: 31853058). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -
Missense variant in a coldspot region where missense variants are very unlikely to be pathogenic (PMID:31911673). -
This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -

Variant summary: BRCA2 c.3517A>T (p.Ile1173Phe) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a benign effect of the variant on protein function. The variant was absent in 250806 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.3517A>T has been reported in the literature in individuals affected with BRCA2-related cancers (examples: Incorvaia_2020, Fanale_2021). These report(s) do not provide unequivocal conclusions about association of the variant with hereditary breast and ovarian cancer syndrome. Co-occurrences with other pathogenic variants have been reported (BRCA1 c.514delC , p.Gln172fs; BRCA1 c.4964_4982del, p.Ser1655fs), providing supporting evidence for a benign role (Fanale_2021). At least one publication reports experimental evidence evaluating an impact on protein function. These results showed no damaging effect of this variant (example: Xia_2022). The following publications have been ascertained in the context of this evaluation (PMID: 32980867, 34178674, 27149842, 32380732, 35762214). ClinVar contains an entry for this variant (Variation ID: 96793). Based on the evidence outlined above, the variant was classified as VUS-possibly benign. -
Breast-ovarian cancer, familial, susceptibility to, 2 Uncertain:2
This missense variant replaces isoleucine with phenylalanine at codon 1173 of the BRCA2 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). To our knowledge, functional studies have not been reported for this variant. This variant has been reported in an individual affected with breast cancer and an individual affected with ovarian cancer (PMID: 34178674). This variant has not been identified in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

not provided Uncertain:1
Not observed in large population cohorts (Lek 2016); In silico analysis supports that this missense variant has a deleterious effect on protein structure/function; Has not been previously published as pathogenic or benign to our knowledge; Also known as 3745A>T -
Hereditary breast ovarian cancer syndrome Uncertain:1
This sequence change replaces isoleucine, which is neutral and non-polar, with phenylalanine, which is neutral and non-polar, at codon 1173 of the BRCA2 protein (p.Ile1173Phe). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with breast cancer, chordoma, and/or ovarian cancer (PMID: 34178674, 35762214). ClinVar contains an entry for this variant (Variation ID: 96793). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is not expected to disrupt BRCA2 protein function with a negative predictive value of 95%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
