In addition, it has been identified in individuals affected with breast/ovarian cancer in the published literature (PMID: 29446198 (2018), 17688236 (2007)). This variant has not been reported in large, multi-ethnic general populations. Based on the available information, this variant is classified as pathogenic. -
Hereditary cancer-predisposing syndrome Pathogenic:2
The c.4551_4554delAGAA pathogenic mutation, located in coding exon 10 of the BRCA2 gene, results from a deletion of four nucleotides at positions 4551 to 4554, causing a translational frameshift with a predicted alternate stop codon (p.K1517Nfs*25). This mutation is located in the ovarian cancer cluster region of the BRCA2 gene and was reported in a family with two cases of ovarian cancer (Ramus SJ et al. Hum. Mutat. 2007 Dec; 28(12):1207-15). In addition to the clinical data presented in the literature, this alteration is expected to result in loss of function by premature protein truncation or nonsense-mediated mRNA decay. As such, this alteration is interpreted as a disease-causing mutation. -

Variant summary: BRCA2 c.4551_4554delAGAA (p.Lys1517AsnfsX25) results in a premature termination codon, predicted to cause a truncation of the encoded protein or absence of the protein due to nonsense mediated decay, which are commonly known mechanisms for disease. Truncations downstream of this position have been classified as pathogenic by our laboratory. The variant was absent in 250542 control chromosomes. c.4551_4554delAGAA has been reported in the literature in at least one individual with ovarian cancer and a family history of ovarian cancer (Ramus_2007, Rebbeck_2018). To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. Four clinical diagnostic laboratories and two expert panels have submitted clinical-significance assessments for this variant to ClinVar after 2014 without evidence for independent evaluation. All classified the variant as pathogenic. Based on the evidence outlined above, the variant was classified as pathogenic. -
