The I36T variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved across species. In silico analysis predicts this variant is probably damaging to the protein structure/function. A missense variant in a nearby residue (A35T) has been reported in the Human Gene Mutation Database in association with tyrosinemia type I (Stenson et al., 2014), supporting the functional importance of this region of the protein. Therefore, this variant is a strong candidate for a pathogenic variant, however the possibility that it is a benign variant cannot be excluded. -

Variant summary: FAH c.107T>C (p.Ile36Thr) results in a non-conservative amino acid change located in the N-terminal domain (IPR015377) of the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 251494 control chromosomes. c.107T>C has been reported in the literature in at least two compound heterozygous individuals affected with Tyrosinemia Type 1 (e.g., McKiernan_2015, Yang_2020 (no PMID)). These data indicate that the variant may be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication was ascertained in the context of this evaluation (PMID: 25564536). ClinVar contains an entry for this variant (Variation ID: 430378). Based on the evidence outlined above, the variant was classified as VUS-possibly pathogenic. -
