This variant disrupts a region of the FH protein in which other variant(s) (p.Asn373Lys) have been determined to be pathogenic (PMID: 33442023; Invitae). The c.1117_1119delAAC variant (also known as p.N373del) is located in coding exon 8 of the FH gene. This variant results from an in-frame AAC deletion at nucleotide positions 1117 to 1119. This results in the in-frame deletion of an asparagine at codon 373. This alteration has been observed in at least one individual with a personal and/or family history that is consistent with HLRCC-related disease (Ambry internal data). This amino acid position is highly conserved in available vertebrate species and the impacted region is critical for protein function (Ambry internal data). Based on internal structural analysis, N373del is deleterious and disrupts the active site of the protein (Ajalla Aleixo MA et al. FEBS J, 2019 May;286:1925-1940; Hicks KG et al. Science, 2023 Mar;379:996-1003). In addition, this alteration is predicted to be deleterious by in silico analysis (Choi Y et al. PLoS ONE. 2012; 7(10):e46688). This variant is considered to be rare based on population cohorts in the Genome Aggregation Database (gnomAD). As such, this alteration is classified as likely pathogenic. -
