Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt MSH6 protein function with a positive predictive value of 80%. The p.P408R variant (also known as c.1223C>G), located in coding exon 4 of the MSH6 gene, results from a C to G substitution at nucleotide position 1223. The proline at codon 408 is replaced by arginine, an amino acid with dissimilar properties. This variant was identified in an ovarian cancer patient from a Canadian cohort of 2870 patients undergoing multi-gene panel testing for suspected hereditary cancer and was classified as a variant of unknown significance (Bhai P et al. Front Genet, 2021 Jul;12:698595). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
