Variant summary

Our verdict is Pathogenic. The variant received 16 ACMG points: 16P and 0B. PS3PM1PM2PM5PP2PP3_StrongPP5

The NM_000238.4(KCNH2):c.1591C>T(p.Arg531Trp) variant causes a missense change. The variant allele was found at a frequency of 0.000000695 in 1,439,356 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. 14/23 in silico tools predict a damaging outcome for this variant. Variant has been reported in ClinVar as Conflicting classifications of pathogenicity (no stars). ClinVar reports functional evidence for this variant: "SCV000827271: Experimental studies have shown that this missense change affects KCNH2 function (PMID:23546015)." and additional evidence is available in ClinVar. Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R531P) has been classified as Likely pathogenic.

Genes affected
KCNH2 (HGNC:6251): (potassium voltage-gated channel subfamily H member 2) This gene encodes a component of a voltage-activated potassium channel found in cardiac muscle, nerve cells, and microglia. Four copies of this protein interact with one copy of the KCNE2 protein to form a functional potassium channel. Mutations in this gene can cause long QT syndrome type 2 (LQT2). Transcript variants encoding distinct isoforms have been identified. [provided by RefSeq, May 2022]

ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 16 ACMG points.

PS3
PS3 evidence extracted from ClinVar submissions: SCV000827271: Experimental studies have shown that this missense change affects KCNH2 function (PMID: 23546015).; SCV004836939: Functional studies have shown that this variant causes trafficking deficiency in cardiomyocytes differentiated from human induced pluripotent stem cells (hiPSCs) and impairs potassium channel function in several in vitro cell systems (PMID: 16166152, 23546015, 36525500).; SCV002768334: "Functional studies have demonstrated altered channel current activation and inactivation (PMID: 16166152, 23546015)."; SCV004014663: A functional study conducted in human cell lines demonstrated that this variant dramatically altered Kv11.1 gating (PMID: 23546015).
PM1
In a hotspot region, there are 4 aminoacids with missense pathogenic changes in the window of +-8 aminoacids around while only 1 benign, 17 uncertain in NM_000238.4
PM2
Very rare variant in population databases, with high coverage;
PM5
Other missense variant is known to change same aminoacid residue: Variant chr7-150951801-C-G is described in ClinVar as Likely_pathogenic. ClinVar VariationId is 3370336.Status of the report is criteria_provided_single_submitter, 1 stars.
PP2
Missense variant in the gene, where a lot of missense mutations are associated with disease in ClinVar. The gene has 158 curated pathogenic missense variants (we use a threshold of 10). The gene has 38 curated benign missense variants. Gene score misZ: 3.3724 (above the threshold of 3.09). Trascript score misZ: 2.4846 (below the threshold of 3.09). GenCC associations: The gene is linked to long QT syndrome 2, short QT syndrome type 1, short QT syndrome, Brugada syndrome, long QT syndrome.
PP3
MetaRNN computational evidence supports a deleterious effect, 0.991
PP5
Variant 7-150951802-G-A is Pathogenic according to our data. Variant chr7-150951802-G-A is described in ClinVar as Conflicting_classifications_of_pathogenicity. ClinVar VariationId is 67218.
