In summary, this variant is a novel missense change with uncertain impact on protein function. It has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be disruptive, but these predictions have not been confirmed by published functional studies. This variant is not present in population databases (ExAC no frequency) and has not been reported in the literature in individuals with a MSH2-related disease. This sequence change replaces arginine with cysteine at codon 35 of the MSH2 protein (p.Arg35Cys). The arginine residue is highly conserved and there is a large physicochemical difference between arginine and cysteine. -

The p.R35C variant (also known as c.103C>T), located in coding exon 1 of the MSH2 gene, results from a C to T substitution at nucleotide position 103. The arginine at codon 35 is replaced by cysteine, an amino acid with highly dissimilar properties. In a massively parallel cell-based functional assay testing susceptibility to a DNA damaging agent, 6-thioguanine (6-TG), this variant was determined to be functionally neutral (Jia X et al. Am J Hum Genet, 2021 Jan;108:163-175). This alteration was identified in an individual diagnosed with prostate cancer (Shao YJ et al. BMC Cancer, 2024 Oct;24:1319). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
