This missense variant replaces threonine with isoleucine at codon 933 of the MSH2 protein. Computational prediction suggests that this variant may not impact protein structure and function (internally defined REVEL score threshold <= 0.5, PMID: 27666373). This variant does not impact MSH2 function in a 6-thioguanine sensitivity assay in haploid human cells (internally defined LOF score threshold <= -1.32, PMID: 33357406). To our knowledge, this variant has not been reported in individuals affected with MSH2-related disorders in the literature. This variant has been identified in 2/279118 chromosomes in the general population by the Genome Aggregation Database (gnomAD). The available evidence is insufficient to determine the role of this variant in disease conclusively. Therefore, this variant is classified as a Variant of Uncertain Significance. -

Variant summary: MSH2 c.2798C>T (p.Thr933Ile) results in a non-conservative amino acid change in the encoded protein sequence. Four of five in-silico tools predict a benign effect of the variant on protein function. The variant allele was found at a frequency of 4e-06 in 247730 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.2798C>T has been reported as a VUS in the literature in at least one individual affected with Colorectal Cancer (e.g. Scott_2022). This report does not provide unequivocal conclusions about association of the variant with Hereditary Nonpolyposis Colorectal Cancer. A co-occurrence with another pathogenic variant has been reported by our lab (BRIP1 c.2392C>T, p.R798*), providing supporting evidence for a benign role. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication has been ascertained in the context of this evaluation (PMID: 36550560). ClinVar contains an entry for this variant (Variation ID: 127640). Based on the evidence outlined above, the variant was classified as uncertain significance. -
