NPC1 (HGNC:7897): (NPC intracellular cholesterol transporter 1) This gene encodes a large protein that resides in the limiting membrane of endosomes and lysosomes and mediates intracellular cholesterol trafficking via binding of cholesterol to its N-terminal domain. It is predicted to have a cytoplasmic C-terminus, 13 transmembrane domains, and 3 large loops in the lumen of the endosome - the last loop being at the N-terminus. This protein transports low-density lipoproteins to late endosomal/lysosomal compartments where they are hydrolized and released as free cholesterol. Defects in this gene cause Niemann-Pick type C disease, a rare autosomal recessive neurodegenerative disorder characterized by over accumulation of cholesterol and glycosphingolipids in late endosomal/lysosomal compartments.[provided by RefSeq, Aug 2009]

DNA sequence analysis of the NPC1 gene demonstrated a sequence change, c.530G>A, in exon 5 that results in an amino acid change, p.Cys177Tyr. The p.Cys177Tyr change affects a highly conserved amino acid residue located in a domain of the NPC1 protein that is known to be functional. In-silico pathogenicity prediction tools (SIFT, PolyPhen2, Align GVGD, REVEL) provide contradictory results for the p.Cys177Tyr substitution. This sequence change has been reported in individual(s) with Niemann-Pick type C both in the homozygous and compound heterozygous states (PMID: 11479732, 32709131, 20718790, 16098014, 28222799). This sequence change has not been described in population databases such as ExAC and gnomAD (dbSNP rs80358252). Collectively, this evidence indicates that this sequence change is likely pathogenic, however functional studies have not been performed to prove this conclusively.
