Genes affected
CASR (HGNC:1514): (calcium sensing receptor) The protein encoded by this gene is a plasma membrane G protein-coupled receptor that senses small changes in circulating calcium concentration. The encoded protein couples this information to intracellular signaling pathways that modify parathyroid hormone secretion or renal cation handling, and thus this protein plays an essential role in maintaining mineral ion homeostasis. Mutations in this gene are a cause of familial hypocalciuric hypercalcemia, neonatal severe hyperparathyroidism, and autosomal dominant hypocalcemia. [provided by RefSeq, Aug 2017]

Significance: Uncertain significance
Review Status: criteria provided, single submitter
Collection Method: clinical testing

This CASR variant (rs202219108) is rare (<0.1%) in a large population dataset (gnomAD: 1/251412 total alleles; 0.0004%; no homozygotes) and has not been reported in the literature, to our knowledge. This variant has been reported in ClinVar (Variation #: 585622). Of three bioinformatics tools queried, two predict that the substitution would be damaging while one predicts that it would be tolerated. The leucine at this position is evolutionarily conserved across many of the species assessed. We consider the clinical significance of c.3040C>T to be uncertain at this time. -

Nephrolithiasis/nephrocalcinosis Uncertain:1

Ambry Genetics
Significance: Uncertain significance
Review Status: criteria provided, single submitter
Collection Method: clinical testing

The p.L1014F variant (also known as c.3040C>T), located in coding exon 6 of the CASR gene, results from a C to T substitution at nucleotide position 3040. The leucine at codon 1014 is replaced by phenylalanine, an amino acid with highly similar properties. This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
