Variant summary: CFTR c.638G>T (p.Gly213Val) results in a non-conservative amino acid change located in the ABC transporter type 1, transmembrane domain (IPR011527) of the encoded protein sequence. Four of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 251458 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.638G>T has been reported in the literature in an individual(s) affected with Cystic Fibrosis (De Watcher_2017). This report does not provide unequivocal conclusions about association of the variant with Cystic Fibrosis. At least one publication reports experimental evidence evaluating an impact on protein function. The most pronounced variant effect resulted in approximately 72% of normal chloride channel conductance relative to wild type (Bihler_2024). The following publications have been ascertained in the context of this evaluation (PMID: 28830496, 38388235). No submitters have cited clinical-significance assessments for this variant to ClinVar. Based on the evidence outlined above, the variant was classified as uncertain significance. -

The p.G213V variant (also known as c.638G>T), located in coding exon 6 of the CFTR gene, results from a G to T substitution at nucleotide position 638. The glycine at codon 213 is replaced by valine, an amino acid with dissimilar properties. This variant was identified in an individual with pancreatic sufficiency and a normal sweat chloride level (De Wachter E et al. Orphanet J Rare Dis, 2017 Aug;12:142). Functional studies suggest this variant is responsive to the CFTR modulator combination of elexacaftor, tezacaftor, and ivacaftor with an increased chloride transport function compared to wildtype (Bihler H et al. J Cyst Fibros, 2024 Feb;21:S1569-1993(24)00021-3). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Based on the available evidence, the clinical significance of this variant remains unclear. -
