Genes affected
PKD1 (HGNC:9008): (polycystin 1, transient receptor potential channel interacting) This gene encodes a member of the polycystin protein family. The encoded glycoprotein contains a large N-terminal extracellular region, multiple transmembrane domains and a cytoplasmic C-tail. It is an integral membrane protein that functions as a regulator of calcium permeable cation channels and intracellular calcium homoeostasis. It is also involved in cell-cell/matrix interactions and may modulate G-protein-coupled signal-transduction pathways. It plays a role in renal tubular development, and mutations in this gene cause autosomal dominant polycystic kidney disease type 1 (ADPKD1). ADPKD1 is characterized by the growth of fluid-filled cysts that replace normal renal tissue and result in end-stage renal failure. Splice variants encoding different isoforms have been noted for this gene. Also, six pseudogenes, closely linked in a known duplicated region on chromosome 16p, have been described. ACMG classification

Classification was made for transcript

Our verdict: Pathogenic. The variant received 12 ACMG points.

PVS1
Splicing +-2 bp (donor or acceptor) variant, product NOT destroyed by NMD, known LOF gene, truncates exone, which is 0.022537176 fraction of the gene. Cryptic splice site detected, with MaxEntScore 3.3, offset of -35, new splice context is: gcagggccccgccccggcAGcgt. Cryptic site results in frameshift change. If cryptic site found is not functional and variant results in exon loss, it results in inframe change.
PM2
Very rare variant in population databases, with high coverage;
PP5
Variant 16-2091175-C-T is Pathogenic according to our data. Variant chr16-2091175-C-T is described in ClinVar as Pathogenic/Likely_pathogenic. ClinVar VariationId is 440137.Status of the report is criteria_provided_multiple_submitters_no_conflicts, 2 stars.

This variant is classified as Pathogenic. Evidence in support of pathogenic classification: Canonical splice site variant without proven consequence on splicing (no functional evidence available); Variant is absent from gnomAD (v2, v3 and v4); This variant has strong previous evidence of pathogenicity in unrelated individuals. It has been classified as pathogenic and likely pathogenic by clinical laboratories in ClinVar, and reported in the literature in individuals with polycystic kidney disease (PMID: 31740684, 30989420); Another splice variant comparable to the one identified in this case has limited previous evidence for pathogenicity. c.11713-2A>G is classified as pathogenic in ClinVar, and was reported in the literature in an affected father and child with ADPKD (PMID: 26139440); Abnormal splicing is predicted by in silico tool and affected nucleotide is highly conserved. Additional information: This variant is heterozygous; This gene is associated with autosomal dominant disease. Polycystic kidney disease 1 (MIM#173900) is predominantly caused by monoallelic variants, with rare reports of biallelic variants causing disease (OMIM); An alternative nucleotide change at the same canonical splice site is observed in gnomAD (v4: 1 heterozygote(s), 0 homozygote(s)); Loss of function is a known mechanism of disease in this gene and is associated with polycystic kidney disease 1 (MIM#173900); Inheritance information for this variant is not currently available in this individual.
