The encoded protein is one of the components of a calcium channel, composed of a tetramer of the ryanodine receptor proteins and a tetramer of FK506 binding protein 1B proteins, that supplies calcium to cardiac muscle. A variant of uncertain significance has been identified in the RYR2 gene. The P1578R variant has not been published as a pathogenic variant, nor has it been reported as a benign variant to our knowledge. This variant was not observed in approximately 6,500 individuals of European and African American ancestry in the NHLBI Exome Sequencing Project, indicating it is not a common benign variant in these populations. The P1578R variant is a non-conservative amino acid substitution, which is likely to impact secondary protein structure as these residues differ in polarity, charge, size and/or other properties. This substitution occurs at a position that is conserved in mammals and in silico analysis predicts this variant is probably damaging to the protein structure/function. However, the P1578R variant is not located in one of the three hot-spot regions of the RYR2 gene, where the majority of pathogenic missense variants occur (Medeiros-Domingo et al., 2009). -

The RYR2 c.4733C>G; p.Pro1578Arg variant (rs765351308), to our knowledge, is not reported in the medical literature but is reported in ClinVar (Variation ID: 373788). This variant is only observed on six alleles in the Genome Aggregation Database, indicating it is not a common polymorphism. The proline at codon 1578 is highly conserved, and computational analyses (SIFT, PolyPhen-2) predict that this variant is deleterious.
