NM_001035.3:c.4912T>A
Variant summary
Our verdict is Likely benign. The variant received -4 ACMG points: 1P and 5B. PP2BP4BS2
The NM_001035.3(RYR2):c.4912T>A(p.Ser1638Thr) variant causes a missense, splice region change. The variant allele was found at a frequency of 0.00000754 in 1,458,474 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a benign outcome for this variant. 3/3 splice prediction tools predict no significant impact on normal splicing. Variant has been reported in ClinVar as Uncertain significance (★★). Synonymous variant affecting the same amino acid position (i.e. S1638S) has been classified as Likely benign.

not provided Uncertain:1
Not observed at significant frequency in large population cohorts (gnomAD); In silico analysis supports that this missense variant does not alter protein structure/function; Not located in one of the three hot-spot regions of the RYR2 gene, where the majority of pathogenic missense variants occur (Medeiros-Domingo et al., 2009); Has not been previously published as pathogenic or benign to our knowledge -
Catecholaminergic polymorphic ventricular tachycardia 1 Uncertain:1
In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. Algorithms developed to predict the effect of missense changes on protein structure and function (SIFT, PolyPhen-2, Align-GVGD) all suggest that this variant is likely to be tolerated, but these predictions have not been confirmed by published functional studies and their clinical significance is uncertain. This variant has not been reported in the literature in individuals with RYR2-related disease. ClinVar contains an entry for this variant (Variation ID: 201252). This variant is not present in population databases (ExAC no frequency). This sequence change replaces serine with threonine at codon 1638 of the RYR2 protein (p.Ser1638Thr). The serine residue is highly conserved and there is a small physicochemical difference between serine and threonine. -
Arrhythmogenic right ventricular dysplasia 1 Uncertain:1

Cardiovascular phenotype Uncertain:1
The p.S1638T variant (also known as c.4912T>A), located in coding exon 37 of the RYR2 gene, results from a T to A substitution at nucleotide position 4912. The serine at codon 1638 is replaced by threonine, an amino acid with similar properties. This amino acid position is well conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
