IL18BP (HGNC:5987): (interleukin 18 binding protein) The protein encoded by this gene functions as an inhibitor of the proinflammatory cytokine, IL18. It binds IL18, prevents the binding of IL18 to its receptor, and thus inhibits IL18-induced IFN-gamma production, resulting in reduced T-helper type 1 immune responses. This protein is constitutively expressed and secreted in mononuclear cells. Elevated level of this protein is detected in the intestinal tissues of patients with Crohn's disease. This alteration results from a T to C substitution at nucleotide position 19, causing the tryptophan (W) at amino acid position 7 to be replaced by an arginine (R). This sequence change replaces tryptophan, which is neutral and slightly polar, with arginine, which is basic and polar, at codon 7 of the IL18BP protein (p.Trp7Arg). This variant is present in population databases (rs373223475, gnomAD 0.006%). This variant has not been reported in the literature in individuals affected with IL18BP-related conditions. An algorithm developed to predict the effect of missense changes on protein structure and function outputs the following: PolyPhen-2: "Benign". The arginine amino acid residue is found in multiple mammalian species, which suggests that this missense change does not adversely affect protein function. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -
