The Ala1870Asp variant in SCN5A has not been reported in individuals with cardio myopathy or in large population studies. Computational analyses (biochemical ami no acid properties, conservation, AlignGVGD, PolyPhen2, and SIFT) suggest that t he Ala1870Asp variant may impact the normal function of the protein, though this information is not predictive enough to determine pathogenicity conclusively. A dditional information is needed to fully assess the clinical significance of thi s variant. -
Variant summary: SCN5A c.5609C>A (p.Ala1870Asp) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 249268 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.5609C>A has been reported in the literature in individuals affected with irritable bowel syndrome and dilated cardiomyopathy (Mazzarotto_2020, McGurk_2023, Beyder_2014). These report(s) do not provide unequivocal conclusions about association of the variant with Brugada Syndrome. At least one publication reports experimental evidence evaluating an impact on protein function, however, does not allow convincing conclusions about the variant effect (Beyder_2014). This variant was reported in individual(s) with features consistent with SCN5A-related arrhythmias and/or cardiomyopathy (Beyder A et al. Gastroenterology, 2014 Jun;146:1659-1668; Mazzarotto F et al. Circulation, 2020 02;141:387-398; Ambry internal data). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -
