Genes affected
SCN5A (HGNC:10593): (sodium voltage-gated channel alpha subunit 5) The protein encoded by this gene is an integral membrane protein and tetrodotoxin-resistant voltage-gated sodium channel subunit. This protein is found primarily in cardiac muscle and is responsible for the initial upstroke of the action potential in an electrocardiogram. Defects in this gene have been associated with long QT syndrome type 3 (LQT3), atrial fibrillation, cardiomyopathy, and Brugada syndrome 1, all autosomal dominant cardiac diseases. Alternative splicing results in several transcript variants encoding different isoforms. [provided by RefSeq, May 2022]

The Ala1870Asp variant in SCN5A has not been reported in individuals with cardio myopathy or in large population studies. Computational analyses (biochemical ami no acid properties, conservation, AlignGVGD, PolyPhen2, and SIFT) suggest that t he Ala1870Asp variant may impact the normal function of the protein, though this information is not predictive enough to determine pathogenicity conclusively. A dditional information is needed to fully assess the clinical significance of thi s variant. -

Variant summary: SCN5A c.5609C>A (p.Ala1870Asp) results in a non-conservative amino acid change in the encoded protein sequence. Three of five in-silico tools predict a damaging effect of the variant on protein function. The variant was absent in 249268 control chromosomes. The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.5609C>A has been reported in the literature in individuals affected with irritable bowel syndrome and dilated cardiomyopathy (Mazzarotto_2020, McGurk_2023, Beyder_2014). These report(s) do not provide unequivocal conclusions about association of the variant with Brugada Syndrome. At least one publication reports experimental evidence evaluating an impact on protein function, however, does not allow convincing conclusions about the variant effect (Beyder_2014). This variant is also known as c.5606C>A. The following publications have been ascertained in the context of this evaluation (PMID: 24613995, 31983221, 37652022). ClinVar contains an entry for this variant (Variation ID: 179280). Based on the evidence outlined above, the variant was classified as uncertain significance. -

This sequence change replaces alanine, which is neutral and non-polar, with aspartic acid, which is acidic and polar, at codon 1870 of the SCN5A protein (p.Ala1870Asp). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with irritable bowel syndrome and suggestive Brugada-type ECG pattern or dilated cardiomyopathy (PMID: 24613995, 31983221, 37652022). This variant is also known as c.5606C>A. ClinVar contains an entry for this variant (Variation ID: 179280). An algorithm developed to predict the effect of missense changes on protein structure and function (PolyPhen-2) suggests that this variant is likely to be disruptive. Experimental studies have shown that this missense change affects SCN5A function (PMID: 24613995). In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance. -

The p.A1870D variant (also known as c.5609C>A), located in coding exon 27 of the SCN5A gene, results from a C to A substitution at nucleotide position 5609. The alanine at codon 1870 is replaced by aspartic acid, an amino acid with dissimilar properties. This variant was reported in individual(s) with features consistent with SCN5A-related arrhythmias and/or cardiomyopathy (Beyder A et al. Gastroenterology, 2014 Jun;146:1659-1668; Mazzarotto F et al. Circulation, 2020 02;141:387-398; Ambry internal data). This amino acid position is highly conserved in available vertebrate species. In addition, the in silico prediction for this alteration is inconclusive. Based on the available evidence, the clinical significance of this variant remains unclear. -
