SCN1A (HGNC:10585): (sodium voltage-gated channel alpha subunit 1) Voltage-dependent sodium channels are heteromeric complexes that regulate sodium exchange between intracellular and extracellular spaces and are essential for the generation and propagation of action potentials in muscle cells and neurons. Each sodium channel is composed of a large pore-forming, glycosylated alpha subunit and two smaller beta subunits. This gene encodes a sodium channel alpha subunit, which has four homologous domains, each of which contains six transmembrane regions. Allelic variants of this gene are associated with generalized epilepsy with febrile seizures and epileptic encephalopathy. Alternative splicing results in multiple transcript variants. The RefSeq Project has decided to create four representative RefSeq records. Three of the transcript variants are supported by experimental evidence and the fourth contains alternate 5' untranslated exons, the exact combination of which have not been experimentally confirmed for the full-length transcript. The c.1076A>G (p.N359S) alteration is located in exon 8 (coding exon 8) of the SCN1A gene. This alteration results from an A to G substitution at nucleotide position 1076, causing the asparagine (N) at amino acid position 359 to be replaced by a serine (S). for SCN1A-related seizure disorders; however, its clinical significance for SCN1A-related hemiplegic migraine is uncertain. This variant was not reported in population-based cohorts in the Genome Aggregation Database (gnomAD). This variant has been detected in multiple individuals with features consistent with SCN1A-related seizure disorders, including multiple cases of reported de novo occurrence (Zuberi, 2011; Gaily, 2013; Butler, 2017; Chahal, 2021; Brunklaus, 2022; Veltra, 2024). This amino acid position is well conserved in available vertebrate species. This missense alteration is located in a region that has a low rate of benign missense variation (Lek, 2016; Firth, 2009). The in silico prediction for this alteration is inconclusive. Based on the available evidence, this alteration is classified as pathogenic. -

This sequence change replaces asparagine, which is neutral and polar, with serine, which is neutral and polar, at codon 359 of the SCN1A protein (p.Asn359Ser). This variant is not present in population databases (gnomAD no frequency). This missense change has been observed in individual(s) with Dravet syndrome (PMID: 21248271). In at least one individual the variant was observed to be de novo. ClinVar contains an entry for this variant (Variation ID: 189864). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt SCN1A protein function with a positive predictive value of 95%. This variant disrupts the p.Asn359 amino acid residue in SCN1A. Other variant(s) that disrupt this residue have been observed in individuals with SCN1A-related conditions (PMID: 23708187, 23808377, 29142202), which suggests that this may be a clinically significant amino acid residue. For these reasons, this variant has been classified as Pathogenic. -
