Variant summary: TTN c.42373G>A (p.Val14125Ile) results in a conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function are either unavailable or do not agree on the potential impact of this missense change. The variant allele was found at a frequency of 2e-05 in 246106 control chromosomes (gnomAD). The available data on variant occurrences in the general population are insufficient to allow any conclusion about variant significance. c.42373G>A has been observed in one individual affected with hypertrophic cardiomyopathy (Lopes_2013). The report does not provide unequivocal conclusions about association of the variant with Autosomal Recessive Titinopathy. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported. The following publication have been ascertained in the context of this evaluation (PMID: 23396983). ClinVar contains an entry for this variant (Variation ID: 195639). The p.V7628I variant (also known as c.22882G>A), located in coding exon 93 of the TTN gene, results from a G to A substitution at nucleotide position 22882. The valine at codon 7628 is replaced by isoleucine, an amino acid with highly similar properties. This variant has been reported in a hypertrophic cardiomyopathy cohort in conjunction with variants in other cardiac-related genes (Lopes LR et al. J. Med. Genet., 2013 Apr;50:228-39). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be tolerated by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
