Genes affected
DES (HGNC:2770): (desmin) This gene encodes a muscle-specific class III intermediate filament. Homopolymers of this protein form a stable intracytoplasmic filamentous network connecting myofibrils to each other and to the plasma membrane. Mutations in this gene are associated with desmin-related myopathy, a familial cardiac and skeletal myopathy (CSM), and with distal myopathies. [provided by RefSeq, Jul 2008]

Available data are insufficient to determine the clinical significance of the variant at this time. The frequency of this variant in the general population is higher than would generally be expected for pathogenic variants in this gene. (http://gnomad.broadinstitute.org) Computational tools predict that this variant is damaging. -

The DES c.785A>T; p.Glu262Val variant (rs147327878, ClinVar Variation ID: 44270) is reported in an individual with reduced ejection fraction and arrhythmia who also has a family history of cardiomyopathy and sudden death (Pugh 2014). This variant is observed in the African/African American population with an allele frequency of 0.26% (64/24820 alleles) in the Genome Aggregation Database (v2.1.1). Computational analyses predict that this variant is deleterious (REVEL: 0.855). Due to limited information, the clinical significance of this variant is uncertain at this time. Variant summary: DES c.785A>T (p.Glu262Val) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant allele was found at a frequency of 0.00025 in 280670 control chromosomes, predominantly at a frequency of 0.0026 within the African or African-American subpopulation in the gnomAD database. The observed variant frequency within African or African-American control individuals in the gnomAD database is approximately 1.04 fold of the estimated maximal expected allele frequency for a pathogenic variant in DES causing Autosomal Recessive Desminopathy phenotype (0.0025). To our knowledge, no occurrence of c.785A>T in individuals affected with Autosomal Recessive Desminopathy and no experimental evidence demonstrating its impact on protein function have been reported. ClinVar contains an entry for this variant (Variation ID: 44270). Based on the evidence outlined above, the variant was classified as likely benign. -

Variant classified as Uncertain Significance - Favor Benign. The p.Glu262Val var iant in DES has been identified by our laboratory in 3 African American individu als with features of DCM but is also present in 0.2% (25/10192) of African chrom osomes in the Exome Aggregation Consortium database (ExAC, http://exac.broadinst itute.org; dbSNP rs147327878). Computational prediction tools and conservation a nalysis suggest that the p.Glu262Val variant may impact the protein, though this information is not predictive enough to determine pathogenicity. In summary, wh ile the clinical significance of the p.Glu262Val variant is uncertain, its frequ ency suggests that it is more likely to be benign. -

This alteration is classified as likely benign based on a combination of the following: seen in unaffected individuals, population frequency, intact protein function, lack of segregation with disease, co-occurrence, RNA analysis, in silico models, amino acid conservation, lack of disease association in case-control studies, and/or the mechanism of disease or impacted region is inconsistent with a known cause of pathogenicity. -
