POLG (HGNC:9179): (DNA polymerase gamma, catalytic subunit) Mitochondrial DNA polymerase is heterotrimeric, consisting of a homodimer of accessory subunits plus a catalytic subunit. The protein encoded by this gene is the catalytic subunit of mitochondrial DNA polymerase. The encoded protein contains a polyglutamine tract near its N-terminus that may be polymorphic. Defects in this gene are a cause of progressive external ophthalmoplegia with mitochondrial DNA deletions 1 (PEOA1), sensory ataxic neuropathy dysarthria and ophthalmoparesis (SANDO), Alpers-Huttenlocher syndrome (AHS), and mitochondrial neurogastrointestinal encephalopathy syndrome (MNGIE). Variant summary: POLG c.2596C>T (p.Arg866Trp) results in a non-conservative amino acid change in the encoded protein sequence. Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. Consensus agreement among computation tools predict no significant impact on normal splicing. However, these predictions have yet to be confirmed by functional studies. The variant allele was found at a frequency of 1.6e-05 in 251350 control chromosomes (gnomAD). c.2596C>T has been observed in individuals affected with bipolar disorder or Parkinsons disease (Kasahara_2017, Zhao_2020). These report(s) do not provide unequivocal conclusions about association of the variant with POLG-Related Spectrum Disorders. At least one publication reports experimental evidence evaluating an impact on protein function and this variant affected the POLG protein function (Kasahara_2017). The following publications have been ascertained in the context of this evaluation (PMID: 27987238, 32613234). ClinVar contains an entry for this variant (Variation ID: 502515). The p.R866W variant (also known as c.2596C>T), located in coding exon 15 of the POLG gene, results from a C to T substitution at nucleotide position 2596. The arginine at codon 866 is replaced by tryptophan, an amino acid with dissimilar properties. In one study, this alteration was detected in 2/796 Japanese individuals with bipolar disorder and in 1/767 controls (Kasahara T et al. Psychiatry Clin. Neurosci., 2017 Aug;71:518-529). This amino acid position is highly conserved in available vertebrate species. In addition, this alteration is predicted to be deleterious by in silico analysis. Since supporting evidence is limited at this time, the clinical significance of this alteration remains unclear. -
