POLG (HGNC:9179): (DNA polymerase gamma, catalytic subunit) Mitochondrial DNA polymerase is heterotrimeric, consisting of a homodimer of accessory subunits plus a catalytic subunit. The protein encoded by this gene is the catalytic subunit of mitochondrial DNA polymerase. The encoded protein contains a polyglutamine tract near its N-terminus that may be polymorphic. Defects in this gene are a cause of progressive external ophthalmoplegia with mitochondrial DNA deletions 1 (PEOA1), sensory ataxic neuropathy dysarthria and ophthalmoparesis (SANDO), Alpers-Huttenlocher syndrome (AHS), and mitochondrial neurogastrointestinal encephalopathy syndrome (MNGIE). This sequence change replaces alanine, which is neutral and non-polar, with valine, which is neutral and non-polar, at codon 957 of the POLG protein (p.Ala957Val). This variant is present in population databases (rs753160398, gnomAD 0.003%). This missense change has been observed in individuals with clinical features of autosomal recessive POLG-related conditions (PMID: 21880868, 25466440, 32703289). ClinVar contains an entry for this variant (Variation ID: 280016). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is expected to disrupt POLG protein function with a positive predictive value of 95%. This variant disrupts the p.Ala957 amino acid residue in POLG. Other variant(s) that disrupt this residue have been determined to be pathogenic (PMID: 12210792, 15258572, 15689359, 23208208, 29915382). Algorithms developed to predict the effect of missense changes on protein structure and function all suggest that this variant is likely to be disruptive. The variant allele was found at a frequency of 8e-06 in 251232 control chromosomes. c.2870C>T has been observed in the compound heterozygous state in individuals affected with Mitochondrial DNA Depletion Syndrome - POLG Related (Yamazaki_2014, Tang_2011). These data indicate that the variant is likely to be associated with disease. To our knowledge, no experimental evidence demonstrating an impact on protein function has been reported.
