Variant summary
Our verdict is Pathogenic. The variant received 14 ACMG points: 14P and 0B. PM2PM5PP3_ModeratePP5_Very_Strong
The NM_003907.3(EIF2B5):c.584G>A(p.Arg195His) variant causes a missense change involving the alteration of a conserved nucleotide. The variant allele was found at a frequency of 0.0000173 in 1,614,198 control chromosomes in the GnomAD database, with no homozygous occurrence. In-silico tool predicts a pathogenic outcome for this variant. Variant has been reported in ClinVar as Pathogenic (★★). Another variant affecting the same amino acid position, but resulting in a different missense (i.e. R195C) has been classified as Likely pathogenic.

Variant summary: EIF2B5 c.584G>A (p.Arg195His) results in a non-conservative amino acid change in the encoded protein sequence. Five of five in-silico tools predict a damaging effect of the variant on protein function. The variant allele was found at a frequency of 2e-05 in 251394 control chromosomes. c.584G>A has been observed in multiple homozygous and compound heterozygous individuals affected with Leukoencephalopathy With Vanishing White Matter (e.g., Fogli_2002, Fogli_2004). The following publications have been ascertained in the context of this evaluation (PMID: 12325082, 15054402). ClinVar contains an entry for this variant (Variation ID: 5946). Based on the evidence outlined above, the variant was classified as pathogenic. -

For these reasons, this variant has been classified as Pathogenic. Experimental studies have shown that this missense change affects EIF2B5 function (PMID: 19023445). Advanced modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) performed at Invitae indicates that this missense variant is expected to disrupt EIF2B5 protein function. ClinVar contains an entry for this variant (Variation ID: 5946). This missense change has been observed in individuals with EIF2B5-related conditions (PMID: 12325082, 15136673). It has also been observed to segregate with disease in related individuals. This variant is present in population databases (rs113994054, gnomAD 0.008%). This sequence change replaces arginine, which is basic and polar, with histidine, which is basic and polar, at codon 195 of the EIF2B5 protein (p.Arg195His). -
