Genes affected
FGF14 (HGNC:3671): (fibroblast growth factor 14) The protein encoded by this gene is a member of the fibroblast growth factor (FGF) family. FGF family members possess broad mitogenic and cell survival activities, and are involved in a variety of biological processes, including embryonic development, cell growth, morphogenesis, tissue repair, tumor growth and invasion. A mutation in this gene is associated with autosomal dominant cerebral ataxia. Alternatively spliced transcript variants have been found for this gene. [provided by RefSeq, Jul 2008]

ACMG classification

Classification was made for transcript

Our verdict: Likely_benign. The variant received -3 ACMG points.

BP4
Computational evidence support a benign effect (MetaRNN=0.2060414).
BS1
Variant frequency is greater than expected in population mid. GnomAdExome4 allele frequency = 0.0000137 (20/1461250) while in subpopulation MID AF = 0.000694 (4/5760). AF 95% confidence interval is 0.000236. There are 0 homozygotes in GnomAdExome4. There are 10 alleles in the male GnomAdExome4 subpopulation. Median coverage is 30. This position passed quality control check. Existence of Clinvar submissions makes me limit the strength of this signal to Supporting

This sequence change replaces histidine, which is basic and polar, with asparagine, which is neutral and polar, at codon 86 of the FGF14 protein (p.His86Asn). This variant is present in population databases (rs758950207, gnomAD 0.006%). This variant has not been reported in the literature in individuals affected with FGF14-related conditions. ClinVar contains an entry for this variant (Variation ID: 585855). Invitae Evidence Modeling of protein sequence and biophysical properties (such as structural, functional, and spatial information, amino acid conservation, physicochemical variation, residue mobility, and thermodynamic stability) indicates that this missense variant is not expected to disrupt FGF14 protein function with a negative predictive value of 80%. In summary, the available evidence is currently insufficient to determine the role of this variant in disease. Therefore, it has been classified as a Variant of Uncertain Significance.

Available data are insufficient to determine the clinical significance of the variant at this time. The frequency of this variant in the general population is uninformative in assessment of its pathogenicity (http://gnomad.broadinstitute.org). This variant has been seen where an alternate explanation for disease was also identified, suggesting this variant may not cause disease. Computational tools predict that this variant is not damaging.

The c.256C>A (p.H86N) alteration is located in exon 2 (coding exon 2) of the FGF14 gene. This alteration results from a C to A substitution at nucleotide position 256, causing the histidine (H) at amino acid position 86 to be replaced by an asparagine (N). Based on insufficient or conflicting evidence, the clinical significance of this alteration remains unclear.
